Begrijpelijk, want hij had geen enkele ervaring in het buitenland. Daarna ging hij reizen. Hij deed tropenervaring op bij een project in de Filipijnen en kon in 1989 aan de slag bij Artsen Zonder Grenzen. Ze hadden een soort manusje van alles nodig voor een vluchtelingenkamp in Oeganda. Hij moest voedsel mee distribueren, maar ook waterpompen en generatoren onderhouden. Dat kon hij allemaal, mede dankzij het werk dat hij vanaf zijn twaalfde na schooltijd op de boerderij had gedaan. Koene groeide snel door naar hogere functies.
